If your child has ever qualified for and/or received additional services such as occupational therapy, speech and language therapy, physical therapy, or any other kind of therapeutic service, please use the Doc Upload button to provide us with:

1. The initial evaluation report conducted by the specialist who evaluated your child

2. The most recent progress report provided by the specialist working directly with your child
